Nutritionist
Specialty Care
Contribute to the overall well being through the provision of Medical Nutrition Therapy. Under direct supervision, assesses nutrition status of normal to high-risk patients and develops appropriate nutrition care regiments.
Food and nutrition experts with a baccaluareate, master or doctorate degree in nutrition and related sciences. Graduate of a supervised dietetic internship or equivalent. Nationally credentialed by the Commision on Dietetic Registration as an "RD" by successful completion of a national examination and mandatory continuing education. Meet basic qualifications of the registered dietitian. Ongoing continuing education as well as medical updates.

Social Worker
Healthy Steps
The Healthy Steps Specialists are the primary child development resources for mothers and fathers in the practice. The Healthy Steps Specialist is the person in the practice responsible for delivering many services and coordination the efforts of all members of the Healthy Steps team to implement "enhanced strategies." Each healthy Steps Specialist will serve Healthy Steps families in collaboration with a physician or pediatric nurse practitioner.
A master degree and or master degree with license, a bachelor's degree with advanced training or education in child development, family studies, nursing, psychology, or related field. Experience and knowledge about early child growth and development, parent-child relationshiops, child health and family systems. Experience assessing the growth and developing of infants and children under three years of age. Experience working with mothers and fathers; experience facilitating parents group preferred. Demonstrates flexibility to work on and interdisciplinary team; ability to work in various roles and capacities and to respect boundaries in each role.

Patient Care Assistant
Nursing
This position requires a person with training and/or experience in an outpatient clinic or office setting to support the physicians and / or mid-level practitioners in delivering primary care services to patients in an ambulatory care setting. This position is intended to enhance the efficiency with which the practitioner can see the patients. The Medical Assistant is expected to anticipate the needs of the patient and assure the patient and environment is in readiness for the provider.
High school diploma or equivalent (GED). Diploma from an accredited technical Medical Assistant Institution. (Certification required). Previous experience working in outpatient or office setting. CPR certification.

Phlebotomist
Laboratory
This position requires an individual with special training and experience in assisting the Laboratory Supervisor in phlebotomy services and coordination of laboratory activities.
High School graduate or equivalent (GED). Certification as a laboratory assistant. Minimum of one-year phlebotomy experience in a drawing facility.

Healthy Steps
The Healthy Steps Assistants are the secondary child development resource for mothers and fathers in the practice. The Healthy Steps Assistant helps the Healthy Steps Specialist when necessary: completing home visits, office well visits and assisting with special program projects. The Healthy Steps Assistant will serve Healthy Steps families in collaboration with a physician or pediatric nurse practitioner.
High School Diploma or equivalent (GED). Computer Knowledge, previous experience in the medical field.

LPN-Medical Home Coordinator
Medical Home Team
Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals. Ensure that patient tests are appropriate and necessary and are carried out within the established timeframe and that results are promptly available. Communicates with physcians at regular intervals and develops an effective working relationship. Assist physicians to maintain appropriate cost, case, and desired patient outcomes.Introduces self to patient and family and explains medical home coordinator role and process for patient and family to contact medical home coordinator.
Must have active LPN License and have current BLS.
Must be proficient in computer skills/microsoft word. Will be directly working with an electronic health record.
